Key Responsibilities:- Review and assess Individualized Care Plans (ICPs) for assigned patients Conduct outbound follow-ups and handle inbound patient queries with empathy and clarity Coordinate with internal teams for seamless care management Maintain detailed and accurate documentation of all patient interactions What We’re Looking For:- Active NCLEX Certification is a must 0 to 1 year of experience (Freshers welcome!)
