Health Coordinator

Job Description

We are seeking graduates from Science or Allied Health Sciences backgrounds who can effectively serve as a liaison between patients and hospitals by providing telecounseling support following patient discharge. Job role: Conduct post-discharge follow-up calls with patients to monitor their health status and recovery progress. Assess patients for any signs or symptoms related to their recent hospitalization that may require medical attention. Coordinate and schedule teleconsultation services promptly if any concerning symptoms are identified during the follow-up call. Provide patients with clear advice and counseling on post-discharge care, including do’s and don’ts to support optimal recovery. Share additional health tips and guidance to ensure a smooth and safe recovery process for the patient. Maintain accurate documentation of patient interactions. Job location:Ghansoli, Navi Mumbai Shift timings: 9:00 AM to 6:00 PM, Monday to Saturday (Sunday fixed off) Languages Preferred: English, Hindi, and Marathi (Special consideration will be given to candidates proficient in regional languages such as Nagamese, Malayalam, Telugu, Tamil, and Kannada) Job Types: Full-time, Permanent, Fresher Pay: ₹20,000.00 - ₹25,000.00 per month Schedule: Day shift Fixed shift Ability to commute/relocate: Ghansoli, Navi Mumbai, Maharashtra: Reliably commute or planning to relocate before starting work (Required) Willingness to travel: 100% (Required) Work Location: In person Expected Start Date: 31/05/2025
