Responsibilities:Engage with the patients over various touchpoint calls to understand their current medical health status, past medical/surgical history, medications taken, labs done etc.Understand if the patient has any additional requirements medications, labs, dietitian, physiotherapists etc.Understand the patient's current social determinants and lifestyle statusTo effectively engage the patients in giving complete medical history and determine if they have any deviations from the recommended treatment planProvide patients with the psychosocial support needed to cope with chronic, acute or terminal illnessesMonitor patients to evaluate the progress of their health conditions and probe them in the correct direction in case of any alarming symptoms reportedTo smartly handle patient queries and respond to pre-defined FAQs.Understand if the patient is compliant with Labs, Medications, and Diet & coach them effectively to be compliant with the prescribed treatment plan as provided by their treating physicianTo ensure effective patient data managementConduct thorough health assessments to identify patients' health risks, needs, and goals.Develop and implement individualized care plans that address the unique needs of high-risk patients.Provide continuous support, motivation, and education to patients through regular check-ins via phone, video calls, and messaging.Educate patients on risk reduction strategies, healthy lifestyle changes, medication adherence, and self-care techniquesProvide wellness, lifestyle and diet tips wherever deemed necessaryProactively identify if the patient is showing any symptoms or deviation from his care pathTo collaboratively work with the Program Manager in identifying new trends, and new insights and effectively managing patient complianceSkills:Excellent knowledge and interpretation of medical conditions and medical terminologiesEffective Patient medical history taking skillCoaching the patient to be compliant with different medical parametersExcellent communication with clarity in speech; Ability to articulate and talk to the patient clearly without ambiguityActive Listening skills, being compassionate with the patientsPassionate about the role and have patient care as a priorityA natural conversationalist: Should be well-read and can engage patients on a variety of topicsHandling complaints and resolving grievances and conflicts that the patients might haveEntering, transcribing, recording, storing, or maintaining information in written or electronic formPleasant disposition, naturally pleasing to talk and have a positive attitudeCollaboration is key; should be a team player and can bridge the gap across all departments.
